Ortho-Perio Synergy: Enhancing Esthetics and Function Through Interdisciplinary Care

Abstract
Orthodontic therapy—particularly with Clear Aligner Therapy (CAT)—has become an integral part of interdisciplinary dentistry. It enables clinicians to optimize tooth positioning for improved esthetics, restorative access, and implant site development. However, the risk also exists that orthodontic movement can inadvertently cause periodontal harm, including root dehiscence, gingival recession, and marginal bone loss.
Modern digital workflows, including CBCT and intraoral scanning, now allow clinicians to visualize roots, bone, and soft tissue in three dimensions. Yet, AI-driven treatment planning often prioritizes alignment goals over periodontal health, leaving patients—especially those with thin gingival phenotypes—at risk for soft tissue complications.
This lecture presents a proactive, synergistic approach to orthodontic and periodontal therapy. By integrating CAT with the Vestibular Incision Subperiosteal Tunnel Access (VISTA) technique and phenotype modification, clinicians can preserve and enhance soft tissue and bone volume during orthodontic treatment. Practical protocols will be shared to guide clinicians in sequencing orthodontic, surgical, and restorative phases for optimal esthetic and functional outcomes.

Learn the role of soft tissue and alveolar bone phenotype modification in preserving periodontal health during orthodontic therapy
Identify pitfalls and complications associated with aligner therapy, including gingival recession
Mange complex dental implant and periodontal esthetic problems with combination of regenerative periodontal and orthodontic therapy.

Immediate Dentoalveolar Restoration (IDR) Technique:
A Paradigm Shift in the Treatment of Compromised Post-Extraction Sockets in the Esthetic Zone

Abstract
Single-tooth replacement in the esthetic zone has been one of the most common indications for dental implant placement. Compromised teeth are removed using atraumatic principles and immediately replaced by an implant-supported provisional restoration, with excellent esthetic, biological, and functional results.
However, in cases of compromised sites with bone loss/gingival recessions, the clinical scenario presents a different dilemma. Until now, all developed surgical recommendations required long-term treatment with possible undesirable complications in the tissue architecture.
The Immediate Dentoalveolar Restoration (IDR) is a surgical and prosthetical technique established to broaden indications for immediate loading on individual teeth. In this way, tissue losses with varied extensions are reconstructed in the same surgical session of implant placement and provisional crown installation, reducing the number of interventions and keeping predictability on esthetic aspects. The IDR protocol was developed more than 18 years ago from the need to minimize the treatment time and morbidity of reconstructive procedures used in these cases.
In this lecture we discuss the scientific basis, the step-by-step technique and indications. The IDR technique, which advocates minimally invasive surgery, flapless procedures, is presented as a viable and reproducible alternative.
